Testosterone pellets are implanted subcutaneously by a physician every 3 to 6 months and provide a steady, hands-free delivery method. Testosterone undecanoate injections provide 10 to 14 weeks of coverage per injection and are popular in Europe.

Monitoring and Ancillary Medications

Comprehensive TRT management requires regular laboratory monitoring. Key labs typically assessed every 3 to 6 months include total and free testosterone, estradiol, hematocrit and hemoglobin, PSA in men over 40, lipid panel, and comprehensive metabolic panel. Estrogen management is an important aspect of TRT protocols. Testosterone converts to estradiol via aromatase, and elevated estrogen can cause water retention, mood changes, and gynecomastia. Low-dose aromatase inhibitors such as anastrozole are sometimes prescribed when estradiol rises excessively. However, it may stimulate the growth of pre-existing prostate cancer. The saturation model of prostate androgen action suggests that the prostate reaches maximal androgen stimulation at relatively low testosterone concentrations, meaning that restoring testosterone from low to normal levels does not meaningfully increase prostate cancer risk compared to men with already-normal testosterone. This paradigm shift has made TRT accessible to carefully selected men who previously would have been categorically excluded.
